SoonerStart Referral Form
SoonerStart is Oklahoma’s early intervention program designed to meet the needs of families with infants or toddlers (ages birth to 3 years old) with developmental delays and/or disabilities in accordance with the Individuals with Disabilities Education Act (IDEA). The program builds upon and provides supports and resources to assist family members to enhance infants or toddler’s learning and development through every day learning opportunities.
Child Find
Child Find is a public awareness activity to identify, locate, and evaluate children who may have a disability. This applies to children residing in Fort Supply school district, who are ages 3-21 years of age and have not yet graduated from high school. Fort Supply Public School offers full educational opportunities for all students. If you suspect your child may have a disability, or know of a child with a disability who is not receiving a free, appropriate, public education, please contact:
